Physical Therapy Pricing and Insurance at Summus Rehabilitation
Summus Rehabilitation values quality patient care and we spend one to one time with you throughout the duration of your entire treatment. We also believe in continuity of care, meaning during your time with us you will be seeing the same therapist for your entire treatment plan of care. We do not believe insurance should dictate the type of treatment or duration of treatment a patient needs. Our goal is to provide quality care at each visit and get you back on your feet as quickly as possible. Due to the nature of our treatment structure and culture, we do NOT take insurance and are considered an out-of-network provider with ALL insurance companies. If you have out-of-network benefits with your health insurance we will provide you with all of the documentation needed to turn into your insurance company for reimbursement. Therefore payment is due at the time of service.
Summus Rehabilitation is a participating Medicare facility and thus can see Medicare beneficiaries. Summus Rehabilitation requires your insurance card, supplemental card, and ID at the time of your visit and we will bill your insurance directly.
Out-of-network Physical Therapy and Reimbursement
Summus Rehabilitation is considered an out-of-network provider with ALL insurance companies, except for Medicare. We take payment at the time of the visit. If you have out-of-network benefits through your insurance company or a high deductible plan, we will provide you with a superbill and receipt proving payment for the services you receive from Summus. Together these documents can be submitted to your health insurance to go towards a deductible or for reimbursement. We encourage all patients to check with their insurance companies for out of network benefits as they vary with each plan.
Does Medicare Cover Physical Therapy?
Summus Rehabilitation is a participating Medicare facility and thus can see fee for service Medicare beneficiaries. At your first visit, Summus Rehabilitation requires you to bring:
Medicare beneficiary card
Supplemental insurance card
Photo ID
Summus Rehabilitation will bill your insurance directly. For the 2026 year, the Physical therapy and speech therapy combined limit is $2480 with an upper limit of $3000. Each Medicare beneficiary has a deductible that begins on Jan 1. For 2026, the out of pocket deductible for each Medicare beneficiary is $283.00.
Summus Rehabilitation Pricing
We offer 60 minute and 90 minute evaluations. 60 minute evaluations are reserved for single body part pain points. 90 minute evaluations are reserved for multiple body parts and pelvic health appointments. Due to the sensitivity and complexity of the pelvis, we want to have enough time to talk, evaluate, and treat you during the first session.
60 minute evaluation: $210
90 minute evaluation: $260
Physical therapy evaluation price
After the evaluation, you will discuss a plan of care with your PT. We typically see patients 1 time per week for 50 minute one on one treatment sessions. Depending on complexity and pain intensity, the patient and PT may agree to begin twice per week and then taper to one time per week. As the patient sees progress, frequency decreases to every other week until they are able to manage symptoms independently.

3-pack treatment session (50 minutes each): $510 for three sessions
Occasionally, patients may require a 30 minute treatment session.
A 30 minute treatment session is $90.
Follow-up Treatment Sessions Pricing
Once you have control of your symptoms, you may talk with your therapist about implementing a maintenance program every 4-6 weeks. This allows for adding or removing or reviewing exercises to continue to maintain health and wellness without pain.
Maintenance 50 minute session: $170
Please note: If it has been more than six weeks since your last clinic visit, we will complete a re-evaluation to reassess your condition and ensure appropriate treatment moving forward.

Thus an average 1 hour visit at Summus Rehabilitation is 4 units which is about $100 to $110 per visit. This equates to about 20 sessions of physical therapy per year. However, this number can vary depending the treatment time and the codes used.
